If you are tested and are found to have substances that are on a list of those that have been deemed (rightly or wrongly)to provide a performance edge, punitive action may be taken against you and your fights may not be sanctioned by the sanctioning bodies or overseen by the governing bodies. If you don't like it and are determined that you really must have substance x in your body, and really must fight, you can go look for underground fights I guess. There's always a choice.
