It wasn't a headbut by Holt, it was technically a headbutt by Torres. Watch it again and you can see Torres actually move forward and in doing so catches his jaw on the top of Holts head. The actual headbutt was delivered by Torres but it was from his jaw to the Holt's scalp. Unfortunate, but it was a mistake by Torres and definitely not a headbutt by Holt. Slow mo the side view for confirmation.
