Pacquiao said he had a scan which shown a tear and was advised to rest which is why he cancelled sparring. That sounds like an evaluation to me.

Fine, I'll concede that this form was filled out incorrectly then.

This doesn't disprove the fact that he had surgery on his shoulder after the Mayweather fight. This also does not conclude that he could not have been injured (or have exacerbated an injury) during the fight.
