Contamination during the testing process. If the technician was handling another substance or had a dirty glove they could contaminate the sample. Accidents happen even with strict protocols.

The likelihood that type of contamination occurring a second time in the lab would be low.

So if the B sample had no traces of the banned substance then how do you explain the A sample? The athlete is pissing in the other collection seconds after as well giving blood

I wouldn't be able to. Ijs bc most guys after their A sample always try to preach their innocence and ask for the B sample. Unless mishandled or some shady stuff happens, I would assume if the A test positive, the B is always gonna test positive too.
