Under WADA rules now Canelo wouldn't have even have failed a test. So WADA basically acknowledge they were wrong that's why they changed the rules.

BJS failed under VADA rules. His license wasn't suspended because of UKAD.

Fury failed under everyone's rules.

Nobody fails any test under VADA rules. VADA adhere to WADA rules, the same as every other testing agency, including UKAD, and they don't make rulings or pass judgements on their test results, they just report them to the relevant commission.

VADA reported the oxylofrine in BJS's sample to the Massachussets State Athletic Commission because, in some circumstances, it is illegal.

It wasn't illegal for BJS to use it during camp, but The MSAC, in their infinite wisdom, decided not to issue him with a licence to fight in their state. If he'd already obtained a US licence before that test was taken, he would have been allowed to fight, because nobody can revoke an athlete's licence for a test result which doesn't break any WADA rules.
