I can't claim to know this as fact, but I would assume that the middle of the hand when a fist is made using a measuring tape for the circumference.

One thing I know is that no matter how you measure it, 15" is outrageous for someone who isn't a giant. I've shaken hands with Vitali Klitschko and George Foreman. Vitali's hands weren't that big at all. George's were.

Thats how I thought they were measured. I've also shaken hands with Foreman and his hands are like catcher's mitts.
