Guys like Cotto, Ward, Mayweather, ect. shave their heads to rock the bald look. As an ex-bald head myself, it's pretty simple waking up every morning and shaving your head. After a while it becomes an easy routine.

Most of these guys have personal barbers on call 24/7. Floyd gets haircuts at 2:00 in the morning if he wants it.
