If the title is vacant and there is a alphabet unification bout for 3/4s of the titles then its for the Ring title (if that policy is still in place), or if its vacant and the top 2 guys in the Ring Divisional Rankings square off then it can be for the title. If #1 in the rankings faces #3 in the rankings, there is actually a committee polled to see if it is worthy of the title being on the line.

Carlos Baldomir was essentially a no-namer who beat Judah. He was awarded the title, rightfully.
In terms of the title, it is awarded to the winner of the number one guy, and the number two guy fighting. In some cases, if the number 2 guy is not far ahead of the three spot guy, then it can be awarded to the guy in the 1 and three spots. Until one of those scenarios takes place, the title isn't awarded to anyone and it remains vacant.

2. any combination of #1, #2 and #3 fighting determines the champ in a vacant division.
They used to award vacant belts to guys who unified IBF/WBA/WBC, but they changed that or Darch would have a Ring belt. Vacancies can be filled by #1 and #3 fightin as long as #2 don't have a win in the division over either of them.
the no-name wins the belt and for a vacant belt the no1 and no2 in the ring ranking must fight each other (or in some cases 1 and 3, but i dont know what circumstances have to be in play for that to happen)
