What if boxers wore tape or even cloth (even very slightly padded) around the TOP of their fore-heads to prevent the impact of cuts from an "accidental" clashing heads?
Is that a totally crazy idea?

Tape or cloth may actually increase the chances of getting a cut from clashing heads, nor would it be practical because the danger area you would wanna protect would be around the eyelids, not the forehead.
I'd prefer the acceptance of instant replay over some new legalized head protection. A bad call can ruin a fighter's chance, even his career. Wearing tape on your head to protect from cuts is slightly less practical..
