dont think about actually hittin it each time.
its all about rythm. hold your elbow parallel to your shoulder and just circle your arm. once u find the flow u wont even have to look at the bag. ive got a ****ty vid on the net and about half way through there is a guy doing a small amount of speed bag.. http://www.youtube.com/watch?v=VV8RnXWL3iI

It is a rhythm thing. I started with 5 hits - BAP bap bap bap bap. . . BAP bap bap bap bap and got those going faster, and then "graduated" to three hits between punches, and am in the process of getting those going faster. The trick, I found, is to get in a rhythm, even a slow one at first, and stay there. Just keep it consistent and the speed will come.
Kinda like everything else in boxing. Concentrate on form and keep it consistent, and the speed will come.

Best thing I can tell you is take it slow starting off. Listen to the bag don't watch it. Try to hit the bag square with your punches each and every time. Keep your movements tight, don't let one arm fall as you're hitting with the other. Use small, tight circles. And don't get frustrated!
