Where can i get that application from to fill it out is it online or what?

If you are in the U.S. you get a book through USA Boxing, at every event the officials heading the event record all matches.
If you are in another country you might have a official body that operates the same way .

With that said I would refrain from telling your kids wether there are higher consequences or not for getting in trouble with a license, if you have kids that are potentially getting in trouble, they will have bigger problems than losing a license or dealing with law enforcement, like getting shot, kids doing boxing need guidance regarding their behaviour, if you are teaching kids to box then you need to be advicing them on avoiding conflict and the real dangers of getting into fights.
I have resorted to telling my kids that you never know if someone has jiu jitsu skills and no amount of boxing will help them when someone takes em down and snaps their arm out of socket

you can still get into street fights with a amatuer boxing license./ hell you can still be a pro and can in street fights and nothing "EXTRA" happens, unless you kill someone or something lol.

cops dont have access to the registered boxers database in the U.S. let alone other countries. So they wont even know to charge you with extra charges (if they apply cause youre a boxers) even if they wanted to.
