I think people get hung up on "rules" when it comes to this issue and their opinion will be based on where their bias is. Ie the first Fury-Wilder knockdown. Everyone supporting Wilder says it's back of the head, fury fans say it was a legal shot.
I tend to think it was legal because Fury wasnt aiming for the back of his head, and in certain camera angles it actually looks like the first impact was on his ear, it's not clear but the I'd give Fury the benefit of the doubt because it was a standard right hook which landed the way it did because of the way Wilder moved away. Also Wilder has a habit of complaining about back of head shots sometimes even when they seemed ok. I noticed this in the first round of the first fight and a couple of other fights.
Fact is you have to judge each case on it's own merit and it's not a black and white answer.

they are two completely different scenarios
if a fighter turns his back to an opponent... the ref should probably stop the fight, and often does
seriously, who does that? ... it is totally pointless and extremely dangerous, you are clearly in no position to defend yourself
that is a common trait you see in kids/young guys who are inexperienced, and it is quickly coached out of them... ring-time solves that, experienced kids almost never turn their back to an opponent
you will see experienced fighters do it on occasion... but very rarely, and only if they have been overwhelmed/battered/hurt for the first time... at that point the fight should be stopped
if you cannot defend yourself, it is the ref duty to stop the fight... and you cannot defend yourself if you cannot see the punches coming
it is 100% legal to hook someone in the side of the head if they turn their back, and you absolutely should do it
regarding when a fighter ducks low... a ref will usually let it slide if a fighter ducks into a shot, because the punch is deemed unintentional
The ref DOES allow for punching to the back of the head when that happens. That's why you'll see the ref tell the fighter "you ducked into that" or "you turned into that" when they complain about rabbit punches.
