You're correct that the commission provides the bottles, sealed, etc. It's illegal to use an opaque bottle due to the fact that anything can be snuck into it. It's happened in the past. Panama Lewis got caught on television giving Arron Pryor a mixed bottle. Mic even picked up him saying "Give me the bottle...no the other bottle, the one I mixed."apparently he had crushed up asthma pills since they will basically open up your lungs and have you breathing normal again.
That video right there is why it's against the rules, and why it's a big deal.

Both camps need to agree and sign off. But again, anything could have been in that bottle. Standard Asthma pills have a half life of 4-6 hours. How long after a fight are guys tested? Could have been ANYTHING in the bottle, because why would you risk something that you could have easily gotten signed off for?
