They didn't ask for it at all. They began the process of shooting him up, and the commission stopped it.
"A few hours before he was expected to step in the ring, when Manny's doctors began the process, the Nevada Commission stopped the treatment because it said it was unaware of Manny's shoulder injury.

They didn't disclose the injury because he didn't have one when it was time to fight.
Here is the statement directly from his surgeon.
“Manny was having shoulder pain during his training period. This was several weeks before the fight…it was essentially an aggravation of an old injury. This is pretty standard for fighters, and there is no such thing as pristine tissue in high level athletes. We treated the aggravation (with permission from U.S. Anti-Doping Agency) and he was back to his normal strength going into the fight.”

Asked why he needed the injection if he felt good before the fight, Pacquiao said, “No, I don’t have pain or injury before the fight. I just want to make sure that whatever happened, I’m still there. I’m not saying I’m not 100 percent condition.”

And I thought I made it clear what I meant. Maybe he got used to fighting with toradol when he was in training for the fight. But Toradol isn't even meant to be taken past 5 days, so who knows if he began abusing it. Seems clear to me that everyone is saying he was good to go and even had 2 NSAC doctors check him out beyond his surgeon saying it was all good.

The injury, as I mentioned and showed in the quotation above, was him tweaking his shoulder and it being healed back to normal strength, minus the normal wear and tear of a fighter. He didn't need a surgery at that time. He elected to have it to save face. Mayweather had messed up shoulders and elected to have his surgery after the Berto fight. It should be very clear what happened here.
His shoulders were no worse than would be expected of a boxer of his pedigree and he used it as an excuse for the loss.
