This is the most important. People simply cannot stand controversial decisions, and that, in my experience from talking to plenty of older heads, is what gets them disinterested. When you have a clear winner get shafted because of shady judging, people don't want to invest their time to be told what they saw was wrong.

Personally, I don't really care if the sport is mainstream or not. Having more fans does nothing for me or the sport.

MMA does not have these two problems anywhere near as bad as boxing does, and they are a growing sport while boxing is decreasing in popularity.
MMA has 1 major advantage though. It's a 1 promotion sport in boxing there is no UFC for the fans to point to as being the legit champ and no UFC promoter able to put the best in with the best without drawn out contract talks and all the bs that happens to make a major boxing match.

If boxing had only 1 promotion and all the best knew they had to fight under that promotion to earn the most money then the sport would be a lot like MMA.

A weekly show with prospects and mid level fighters is critical. That's the organic way to build fighters into HBO/Showtime and PPV talents. Mike Tyson built his following with impressive wins on ESPN and USA channel. By the time he debuted on HBO he already had a fanbase.

IMO we need more Boxing Gyms across the country and around the world, better distribution of wealth among fighters, better promotion for the sport and fighters, etc. The sport needs to be nurtured again from the ground up. Young people need a reason to want to get into this sport again.

Also, less belts, less confusion, less choice between opponents for the fighters(ducking), more fighters willing to truly take on ALL comers, more regulation/oversight, and more undisputed champs.
