So if it was a preexisting injury, which he has had for several years according to Arum, then wouldn't he have asked for the medical exemption in the past, if so then that would support the theory for the injury, but it would also mean he would have to know how to fill out the form correctly.

And you keep removing the rest of the quote that NSAC had no proof or confirmation of the injury, so they DENIED his request for medication.
And then to make matters worse, Pacquiao didn't mention the injury on the form!
