That he did, but I'm talking about the current landscape where GGG is after that honour and we all saw what GGG did to Macklin.

GGG is already Cottos mandatory, he won that by defeating Rubio. Murray is ranked as the number 1 contender by the WBC, the winner will be Cottos mandatory. Macklin is fighting for an international title which would put him higher in the rankings. Cotto has been granted 1 voluntary defense before facing his mandatory, so Cotto can defend his title against anyone ranked by the WBC at middle weight. And that is fair because he just got to the weight it makes no sense for him to go and fight the most dangerous middle weight in the world when he just got to the division. Look for Cotto to defend his title in a voluntary defense, move down for a title fight at 154, then move back up to fight his mandatory.

That 154 move seems unlikely to me, I don't see Cotto sticking around for anything other than Canelo/Mayweather fights. (and I don't see any incentive for Floyd beyond a middleweight title, so if Cotto loses/fights at 154 that goes out the window)

You have to pay attention to Arum, Cotto will most likely move down 1lbs to 154 to fight Bradley for a vacant title after Canelo, win or lose. Arum has been talking about this fight since before Cotto defeated Martinez.
