I can see one happening if an orthodox fighter throws a left hook, the southpaw opponent, if close enough, could take the blow to the back of the head if he is a sideways fighter like....say James Toney. In theory. But most of the time, fighters have thier heads leaning forward and tuck in to avoid the blow.

If i remember correctly, most of the punches to the back of the head (in Benn vs McClellan) happened mainly when the two fighters were in a clinch.
If a fighter does duck (particulary below the waist line) there's a good chance that they'll get caught with heavy blows to the back of the head.
I don't think James Toney gets caught on the back of the head though, because he's very clever with his head movement, and most punches tend to scuff off his head as he's always moving away from the punch.
The thing with hitting in the back of the head that is so damaging is that there is a sharp ridge that runs up the back side of your skull. When a fetus is forming and its head is forming it eventually meets at the back of the skull foorming this sharp ridge. I dont recall the proper name of this ridge but it is damaging.
When you take a solid impact to the front of your face, your brain will first bounce against the front of the skull, rebounding with less force back to the ridge were it will sustain some damage.
When you are hit directly in the back of the head the brain collides directly with this ridge, sometimes causing severe if not extreme damage.
First hand I can tell you that these shots will F**K you up. Any ref or organization that is in anyway tolerant of these types of blows is not doing the right thing. We just get to ***** about the calls of the fights after word, but a fighter could be laying dead or in acoma if these actions are still tolerated.

From your impression Rockin' is this enforced equally in foreign territories or do they show leniency towards it in the UK?
In other territories I am not certain what goes down. I just know that in my ring the first shot I see deliberatly to the back of the head I will issue a stern caution. After that I will start taking points.

Nigel Benn threw the hardest punches I ever saw a middleweight throw until abandoning that after the Watson fight, 87-88 he left opponents hospitalised with single shots and you could HEAR the sickening impace of the shots. He brought that back for the McClellan fight, that's what did the damage (McClellan's chin was too good).
