The rest of his body will be severely weight drained as his huge sack will suck all his remaining bodily fluids to it, therefore making him a walking skeleton with a monstrous ball sack that just weighs him down, makes him slower and therefore easier to knock out by Pac who's miniscule sack won't take up as much body weight. This is of course the reason Pac is light and fast with his hands and feet.
Pac also having such a little sack will end up winning him the fight as he won't be as threatened with the low blow tactics of Cotto when he (Cotto) gets in trouble. Cotto will swing for the fences, or well below the fence anyway, when he's in trouble but will have a hard time finding the right spot as Pac's little sac will be tucked down and away from Cotto's sites and Cotto will just end up wailing away at the front of Pac's legs.
