3 things:
1: It probably wasn't a double mouth guard, so 1 row of teeth is enough to get the job done.
2: The ear isn't exactly rock hard.

He spit it out. Someone noticed that he had started the rd without his mouthpiece in Holyfield's corner.

Yeah he didnt start the round with the mouthpiece, and bit Holyfield the first time.
Then the ref made him put it back in, he spit it out and bit him a second time.
