I've never been knocked out, but I reckon sometimes because you're "Not with it" you carry on standing.
Interesting question though. The closest I've been to being knocked out was in a car accident where i rolled the car on its roof, the firemen were laughing when they pulled me out from the car because I pulled the handbrake so the car wouldn't roll, even though the car was on its roof.
