How do you avoid it? I have noticed I can feel that tendon getting sore on the top of my middle knuckle.

I bought these stupid short wraps one time because my long ones were getting worn out and after a couple of weeks I noticed a bit of soreness in my hands. Then I bought more of the long ones and everything was fine again.

Use good gloves and wrap well.

The middle knuckle is vulnerable b/c it gets the brunt of the impact.
I have started using EVA foam I cut to go below the knuckles.

I am going to guess Floyd's knuckles/hand were messed up by lazy wrap job when he was young.
Just like Gerrie Coetzee.
Jackie McCoy blamed Gerrie's dad for it.
