Arthur cant/couldnt control everything/everyone around him. The fight was extended in front of him, and he was man enough to fight on and win. All the surrounding events are unimportant when at the end he is awarded his belt back because of HIS OWN STRENGHT AND COURAGE.

For the record... this isn't the first time a doctor has pointed out that a fight should be stopped yet it continued to go on.... Look at Hopkins Echols II when the doctor says that if he can't lift his arm then they've gotta' stop the fight. Hopkins responds that he wants to keep fighting and doesn't want to lose that way (despite it not being a loss) and continues the fight.
