A lot of people cared, he went to jail for one of those crimes (the other was adjudged justified under American law). Even after that there have been countless articles on the matter, it's been brought up in documentaries and so on.
What that has to do with a boxer, a trained individual who knows how to hurt people, insinuating he would like to kill someone, is beyond me.Comment

More context is how Ali was sadistic in the ring, proclaiming he wanted to damage his opponents' health. I don't remember who he was talking about, but he said he wanted to make someone "pay with their health." Meaning he intended to do lasting damage to somebody's well-being.
