The ref made it clear in the locker room and before the fight that punches on the belt would be legal because kov appeared to be wearing the trunks a lil high. Come on Bat, kov was hurt and didn't want to get ktfo.

Notice how Ward is looking for the referees help because Kovalev bends below the waist. I don't think Kovalev looked for help any of the previous 100 times Ward did that.
