I have a feeling he has emotional or substance abuse problems ala Pavlik.

After the Super Six debacle (diving vs Abraham and refusing to fight his friend Ward), he said he was taking time off for "neurological issues" and although he has fought a couple times since then, he has also pulled out of several matchups.

If they were physical neurological injuries he wouldn't be cleared to fight, so lets assume he's either depressed or boozing.
