My guess is natural causes. He was leaving a hotel in Arizona with his wife and collapsed, before he got to the car. He was pronounced dead on arrival. He was my favorite all-time wrestler. Catchweights have been around even before manny and floyd existed, in case you're unaware. Floyd made a contract with JMM to meet at a catchweight, but made no effort to abide by it and just paid him off. Kinda hopeless if you can't tell the difference.
