I more or less agree with you - it's definitely not OK to call people by a name that they consider an insult - UNLESS one doesn't realize that it's actually an insult! (I'm not sure that this was the case with Kovalev though...).

Skin color is just one way of describing people - in a conversation, you would surely describe someone as "the pale guy", "the red-haired guy", the "bald guy", "the guy with the big nose", etc. These are just normal descriptors. I wouldn't say "hi baldy" to someone's face though - but when referring to him in a conversation with other people, why not?
