A doctor's job is not to see if the boxer is taking punishment because of punches or headbutts, his only job is to protect a fighter, period.
The decision of who wins or loses is taken in this case by the referee. If the ref thinks the fight was stopped because of an accidental headbutt, then they go to scorecards. If he thinks it was an intentional headbutt, then it's a DQ. If it was because of a legal punch then it's a TKO, and if it was because of an illegal punch then it's another DQ.
All those decisions are in the rules and depend on the REFEREE decision, not the doctor.
The doctor can only say if a fighter is able or not to continue fighting, that's his only job.
