What is the difference, if any, between the two?

There is only 1 champ, anybody with a strap is the title holder. The champ is the clear top guy and once they reach that status they have to lose, leave the class, or retire to lose that status.

Technically a titleholder is a world champion but I think you'll find most people will have one guy in mind from each division who the consider THE champion. Its usually the Lineal title holder or the outstanding guy from the division.

Well, Wladimir Klitschko is the real heavyweight champion of the world.

Holds The Ring title which gives him lineal status but he's also stood atop the mountain for years.

Vitali and Povetkin are title holders.

        That's the difference.

World Champ is like my boy SSM taking on Veron Forrest
Title holders are like Manny and Floyd cherry picking their opponents
