Khan falling from the Sky.. Who's to blame?

the blame seems to be spread in lots of directions: Hatton promotions, Sky, Khan's family-based business team and even the Haye vs Harrison fight.. after all things considered, who is ultimately responsible, or is it equally spread across the board?
