It is not out of the question that a man who avoids random testing is on quite possibly on steroids. When you also factor in the extreme poverty from which he came, having his dad eat the family dog etc, It becomes very plausible that the end result is a man who is desperate to rise above his economic straits. How many people do much more than just injecting themselves with steroids for a shot at riches? Think of the women who sell their own children etc. That being said, we must carefully look at the circumference of Manny's head and the fact that he once was a cute little 105lb boxer who is now not only beating 165lb men, but beating them into submission.
