Being afraid of needles is a popular phobia. And thats not the full reason on why Manny Pacquiao doesn't do them, mentally he thinks it physically drains him. And boxing is a mental sport, so he is in the right to not do those bs testings. Boxing already has good enough steroid testings. I mean toney, vargas, and many other boxers get caught. So the system can't be all that bad.
