Cotto's father looked fairly healthy in the build up to the Manny fight. I feel the break up with his brother as trainer and the law suit contributed to a lot of stress . Add to it the beating Cotto took and the sexual harassment case of one of Cotto's employees and you have a recipe for a heart attack.

I understand the uncle did a Billy Graham and was instigating legal proceedings for money even before the Pacman fight. Cotto might feel this piled up the stress on his Dad and himself and was a contributing factor to the heart attack. When legal proceedings start things get really acrimonious and it then becomes very difficult to patch things up but blood is thicker than water. Cotto must be really feeling it right now.
