Why wasn’t Holyfield disqualified for headbutts

I watched a few of his fights and both Tyson fights and both Lewis fights he was coming in with his head trying to cut his opponents so why was this allowed to continue his entire career with no refs ever calling him on his fouls and DQing him

There’s multiple videos and commentary on Holyfield positioning his head to be a weapon in his arsenal. The man blatantly head butted opponents in fact it was his head that caused that Quasimodo swelling on Rahman
