Yes there was a form right by them, you can also print them online. Make sure you keep the receipt too, have to buy them today also.

Meh, there was no form by them and the cashier had no idea what I was talking about. The receipt also doesn't list exactly what I bought, it's just a generic receipt with a total..FML
