Because the best in each weight class normally don't fight the best and when they do, they don't really defend it.

Vazquez was the 122 champ and vacaterd. There's normally 7 or 8. The lower weight classes don't unify.

The Ring's HW title has been vacant since Vitali retired in 2005. Same for 147 and Mayweather's "retirement". You don't get the belt back just by returning to the sport after you've officially retired and relinquished the belt.

The 168 lb. title has been vacant since Calzaghe moved up, and the 175 lb. title has been vacant since Calzaghe retired.

The 154 lb. title has been vacant since Wright moved up.

The 130 lb. title has been vacant since Pacquiao moved up. I think 126 also with Pacquiao having the belt (Barrera) and then moving up.
