I'm no expert so I asked an expert for guidance on the matter and this is the response I got:

Well you know it depends on the circumstance, there are factors to consider, many factors. Was the downed fighter the house fighter, was the downed fighter the money fighter, was he paying and how much was he paying, and how bad are your ******** debts because a referee has to keep the wolves from the door. It's important to be fair. But firm.
In this circumstance I think the best course of action would be to leave Miguel bleeding on the floor for seven minutes while the referee asks other people what to do before disqualifying Margarito and then saying it was all about the safety of the fighter. I'm fair but I'm firm.
So obvious to anyone with any boxing knowledge what so ever that he held the punch back and there was no menace in the punch what so ever, Cotto got his ass handed to him as I predicted, plain and simple over rated like so many fighters out there, no heart and no balls. Jeff (no left hook) Lacy springs to mind.
he got hit because he kneeled at the point margarito threw the punch
he didn't, he kneeled at the point when Margarito was going to hit him with his left, he withdrew that punch and hit him with the right when he was down.
