From what I understand Buster Mathis injured his hand on Joe's own head.

Due to a broken knuckle Buster Mathis couldn't compete for the Olympic team and dropped out. Frazier then took up his place. Mathis originally got the spot after beating Frazier in the Olympic trials final.

It was controversial in that Joe kept getting low blow warnings from Buster wearing his trunks too high and pot shotting Joe. Joe felt that he won but I am not sure if it was controversial outside of his mind.
