Has Cotto ever been the #1 fighter in any division he's fought in?

I don't think Cotto was ever the best fighter in his weight class at any time in his career. There was always someone fighting in the same weight class who could beat him at any point in his entire career.

Been Lineal once and that was at MW which was abit iffy but he was Lineal none the less.

He was the lineal middleweight champ because he beat Martinez at a catchweight of 159 and defended it over a full year later against Geale again at a catchweight of 157. The same Geale GGG had KOed a short time earlier. Then he defended his tittle against Canelo at yet another catchweight of 155 and lost. During his reign as lineal and WBC world middleweight champ he refused to fight GGG who was clearly the real middleweight champ and would have crushed Cotto.

Mayweather retired in Dec 2007 just after Cotto had beaten Mosley and Judah. Could have been no.1 then at WW.

Obviously lineal at MW and a top fighter at 154
