You also have to take into account the economics of it though. As far as I know, SELF was a regular print magazine about women's health? If they started putting obese women on their covers nobody would pay $7 to take it home and learn how to become morbidly obese. It would be laughable financial suicide.

This is their first DIGITAL cover. The whole financial model is different. Digital magazines just want you to be shocked into noticing they exist and increasing their click traffic. They could put a giant photo of a turd on the cover and probably all get raises. The same shock-for-clicks model applies to online political articles, sex tube websites, etc.
