As for the first name vs. last name thing, with me, it really depends on the person. I have some friends I refer to by first name, and some I refer to by last.

The only person I have a nickname for is my brother, and I've been calling him that since we were both teenagers, so it's just habit. Overall I find nicknames totally annoying, though, because they're usually ****ing ******ed things like "Chip" or "Ace".
