First and foremost, as some people already mentioned he has to be the one that wants to get into shape. You are obviously a great son for caring about your father's health though. He should be proud of you.
1. Do everything you can to get him off smoking. The problems it causes (other than cancer and heart disease which is bad enough) are too many to even list.
2. Slight changes in the diet. Smaller portions, introduce healthier snacks in the diet, limit certain high-fat dishes, etc. He is not going to buy into getting back in shape if there is a drastic change in his diet. Once he gets used to the small changes, the bigger changes can come later.
3. Take long walks. As someone mentioned trying to start off running right away will only discourage him. Slowly increase the distances. Then after awhile build up to a slight run/walk. Then a full run over a short distance. Increase the distance as his stamina does.
But like I said, he has to want to change. But if nothing else, get him off those cigarettes! Good luck!

This is the best advice, so far. I'm 57, I box, and I'll just add that you want to start him out REAL slow. Just getting him to stretch each day will be a big improvement. Once he starts to stretch, the body will begin to "wake up" and want more. The bodies all want to be healthy, it's the minds that cause all the problems and will be the most difficult to overcome.
Give your Dad lots of support. Don't get discouraged if he "falls off the wagon" every once & a while. Be there to pick him up and help him get back on track. But don't nag him, wanting it more than he does can be counter-productive. And don't get too radical, try getting him to cut DOWN on his smoking and drinking. If you try to get him to cold turkey, he'll just hate you for how bad he feels, cause he's gonna feel BAD before he starts feelin' good.
If he's got pneumonia, see a doc before you do anything.
And be prepared for your old man to tell you to go f yourself you wise-ass little prick. LOL! Us old guys don't like being told what to do. Good Luck.
