Why is it a term to say big balls in boxing? What if you have small balls but brave? Should the saying be cancelled for body shaming?

I always hear the term big balls in boxing or grow a pair, but what if the guy has small balls and a big Cack but is brave do you think this body shaming should still be allowed in these times or is it hurtful and outdated?
