We read about fighters throughout boxing history being accused of "Ducking" other fighter of their era but which fighter Ducked a fighter "without reasonable doubt" and by that i mean provide evidence to prove it was beyond reasonable doubt.

Well, its quite obvious Floyd ducked the best at 147. I doubt Cotto, Williams, Margarito, Clottey ever ducked Mayweather Jr., the payday is too great.

Lennox Lewis recently said on sky broadcast he never boxed a southpaw as a pro and didnt want nothing to do with michael moorer for that reason he doesnt like fighting southpaws

Jack Johnson ducking Sam Langford. Langford was clearly the best heavyweight in the world while Johnson was champion, and despite what Johnson claimed, there were many, many big money offers from promoters like Hugh McIntosh, all of which were turned down by Johnson. I like Johnson but it's hard to overlook all that when evaluating him.

In return for a loan from the National Sporting Club in order to chase Burns to Australia, Johnson signed this agreement:
Gentlemen: I undertake and agree to carry out my contest with Sam Langford on the 22d of February, 1909, on the same terms and conditions as already arranged with Langford, viz. 1,000 pound purse and one-third of the interest in any bioscopic pictures that may be taken. At the same time allow me to tender my thanks to you for the courtesy you have extended to myself and manager, Mr. Sam Fitzpatrick, while we have been in this country.

There are countless other reports of Johnson being offered big money to face Langford. Johnson's stated minimum purse for a title defence was $30,000. Hugh McIntosh met that offer (New York Times, 26th June 1912), then offered $40,000 (New York Times, 9th August 1912) and then $60,000 for him to face Langford, Jeannette and McVea (New York Times, 12th September 1912) and that's just a few of many similar offers. There was plenty of demand and interest in the fight and Johnson just wasn't interested.
And finally, here's a quote from Johnson himself regarding Langford: "I don't want to fight that little smoke. I'm the first black champion and I'm going to be the last."
Only in Sonnyboy land did Johnson not duck Langford.
