      Possible friendly fire because of ill equipped British forces. Every unit in Iraq and Afghanistan has/had things called Blue Force Trackers. The US themselves have shelved out money to ensure most units in theater have these systems. Besides, a certain level of collateral damage is to be expected. British troops have killed more of their own during the Falklands war than the US has killed British troops in friendly fire in the entire decade+ of war in Afghanistan. Is it acceptable? Absolutely not, but it is a risk that majority of commanders on the ground are aware of and would like to avoid.

        This happens more than you think. I feel bad for the solider that died, war is chaos.
