ACKNOWLEDGEMENT OF RISKS AND RELEASE
Where as, ______ (customer) declares Purofirst Disaster Services, here and after referred to as Purofirst to perform restoration services to said property, whereas in order to complete the restoration effectively and completely, it will be necessary for the identified mold to be properly contained and remediated.
TERMS AND CONDITIONS
1. Notification of Risks. Customer hereby acknowledges that Purofirst Disaster Services has advised customer that the customer may suffer, potential health risks as a result of not remediating the microbial growth from the structure Properly.
2. Opportunity to Seek Medical Opinion. Customer hereby represents that he/she has had the opportunity to seek a medical opinion on the potential health effects concerning the microbial growth that was identified. Customer further represents that he/she accepts the risks as forth set
3. Release. Customer hereby releases and forever discharges Purofirst Disaster Services, it’s agents, principals, owners, shareholders, directors, managers and employees from any and all claims as to injuries, damages, and health problems of any nature, whether known or unknown, suffered as a result of not drying the structure.
The undersigned hereby declares that the terms of this acknowledgement of risks and release have been completely read and are fully understood and voluntarily accepted.

I just had to write up a release for a customer because they do not want us to take care of the mold that was found in their house. Does it sound like it makes sence
